This chapter describes the evolution of the Norwegian municipal chief executive officer (MCEO) from 1980 to 1992 and 2018, when the position was weakened politically but strengthened administratively. Today, the Norwegian MCEO corresponds to Mouritzen and Svara’s council–manager model, making it one of the most powerful in the Nordic region, second only to the Finish MCEO. Despite multiple significant contextual changes in the last two to three decades, the essential role of the Norwegian MCEO has remained remarkably stable, demonstrated in survey data from 1997 to 2017. Nevertheless, the data also show that the biographical characteristics of the MCEO have undergone considerable change during this period, most notably the increase in the proportion of female MCEOs from 7% to 30%. Further, today’s MCEOs are older and more highly educated, especially in disciplines such as economics, administration, and public management, reflecting the strong emphasis on economic management and performance in Norwegian municipalities. Finally, MCEOs are met with increasing expectations to combine innovation and management, cooperate internally and externally, and govern municipal affairs through complex, hybrid networks.

This book analyses the municipal chief executive officer (MCEO) of Nordic municipalities. The Nordic countries operate a distinct model of local government in the context of the welfare state. As there are some variations among the countries, it is important to analyse this model in comparative perspective. In the Nordic countries, the post-war expansion of the welfare state took place primarily in local government, which became the main provider of welfare services and one of the largest employers in these countries. In this chapter, we present the rationale for the book, the Nordic municipal context in which the MCEO works, our research questions, and basic theoretical approaches. Finally, we briefly present the research design, methodologies, and outline of the book.

In this concluding chapter of the book Managing Nordic Local Governments: Paradoxes and Challenges of the Municipal Chief Executive, we present the main findings in relation to the Nordic municipal chief executive officer (MCEO) model of embeddedness. The MCEO role is discussed in relation to each of the seven conditions: the local–national welfare state, the governance model, regional dynamics and disparities, political–administrative organizations, leadership expectations, public servants, and career systems. Each of the seven conditions is put into context with the demands, constraints, and choices of the MCEO position to demonstrate the entangled institutional web of the position of the Nordic MCEO. The chapter concludes by discussing the research questions of the book. The main findings of the book suggest that the fundamentals of the MCEO position remain relatively stable over the study period, demonstrating incremental rather than major changes. In sum, the role of the Nordic MCEO has a long tradition, and its importance does not seem to be diminishing; rather, it is the opposite.

: Building heavily on the Health in All Policies (HiAP) approach, Norway implemented the
Public Health Act in 2012 to reduce social inequalities in health. Local public health coordinators
(PHCs) at municipal levels were seen as tools to provide local intersectoral public health work. In this
study, we examine factors related to intersectoral agency and if intersectoral work is understood as
relevant to securing social justice in local policy outcomes. A national web-based survey in 2019 of all
Norwegian PHCs (n = 428) was conducted with a response rate of 60%. Data were analysed through
multiple linear regression, hierarchical regression modelling and structural equation modelling.
Neither factors relating to community contexts nor individual characteristics were associated with
intersectoral agency. Organisational factors, especially position size, being organised at the top level
and having a job description, were significantly associated with perceptions of intersectoral agency.
PHCs seeing themselves as intersectoral agents also found themselves able to affect annual budgets
and policy outcomes. We conclude that municipal PHC positions can be important HiAP tools in
local public health policies. However, organisational factors affect how PHCs perceive their influence
and role in the municipal organisation and thereby their possibilities to influence local policymaking
through intersectoral agency

Throughout Europe, local health services are increasingly being provided through various forms of inter-municipal cooperation (IMC). One of the most common forms of IMC is when small municipalities delegate the operational responsibility for providing health services to a larger host municipality. However, despite the size asymmetry usually inherent in this type of IMC, this aspect has largely been neglected in the existing literature, which mainly focuses on the size of individual municipalities. Based on data from 97 partner municipalities and 25 host municipalities in Norway, this study examines how varying degrees of size asymmetry between them affect the perceived service quality and loss of autonomy resulting from IMC in health services. From the perspective of the relatively smaller partner municipalities, the results suggest that these are likely to benefit greatly from size asymmetry in terms of improved service quality, although this would appear to be at the expense of losing decision-making autonomy to their host. However, from the perspective of the relatively larger hosts municipalities, this type of asymmetry is likely to affect service quality negatively while having no effect on decision-making autonomy.

Inter-municipal cooperation (IMC) has gained widespread recognition as a beneficial strategy for improving efficiency and quality in the provision of out-of-hours emergency care services (OOH services). Little attention, however, has been given to the additional costs of cooperation and the relational processes through which benefits and costs are likely to result. Based on survey data from 266 (77%) Norwegian municipalities involved in IMC in OOH services in 2015, this study aimed to investigate how the structure (governance form, complexity and stability) and quality (trust and consensus) of cooperation processes interact to influence the perceived outcomes (benefits and costs) of IMC in OOH services. Using Structural equation modeling, we found trust and consensus fully mediated the association between the structure and outcomes of IMC. More specifically, the results suggest that cooperation structures characterized by centralized governance, stability over time, and reduced complexity were likely to enhance the benefits and reduce the costs of IMC through trust and consensus.

Introduction: The Norwegian health care system is well organized within its two main sectors - primary health and long term care on the one hand, and hospitals and specialist services on the other. However, the relation between them lacks mediating structures.
Policy practice: Enhancing coordination between primary and secondary health care has been central in Norwegian health care policy the last decade. In 2003 a committee was appointed to identify coordination problems and proposed a lot of practical and organisational recommendations. It relied on an approach challenging primary and secondary health care in shared geographical regions to take action. However, these proposals were not implemented. In 2008 a new Minister of Health and Care worked out plans under the key term "Coordination Reform". These reform plans superseded and expanded the previous policy initiatives concerning cooperation, but represented also a shift in focus to a regulative and centralised strategy, including new health legislation, structural reforms and use of economic incentives that are now about to be implemented.
Discussion: The article analyses the perspectives and proposals of the previous and the recent reform initiatives in Norway and discusses them in relation to integrated care measures implemented in Denmark and Sweden.

This thesis consists of four articles that deal with the introduction of unitary management from2000 and health enterprise reform from 2002 in Norwegian specialist health care (hospitals). The first of the articles (”Knowledge, professions and management. Knowledge perspectiveson management in Norwegian healthcare”) focuses on the historical traditions and formationsof the old collegiate management structure in Norwegian hospitals and how these collegiateforms were founded on the medical profession’s extensive autonomy, bio-medical knowledgeand health planning. The article also examines how Norwegian nurses in the post-Second Warperiod adopted knowledge from the field of general management in US universities, and howthis knowledge adopted by the nursing profession is an early indication of how the nurse adoptto the present professional management regime in Norwegian health enterprises. This also gavenurses access to management positions and legitimacy as managers within this regime. The second article ( ”State ownership and enterprises, new conditions for management inNorwegian hospitals”) studies the question of whether hospital managers have gained moreautonomy after the introduction of the health enterprise reform. Even though Norwegianauthorities have invested a great deal of effort in reforming hospitals into autonomousenterprises by means of empowered general management, medical management and theprofessional logic still have a strong standing in hospitals. If general management has becomemore important in Norwegian hospitals, this is in the administrative top management, where ithas become centralised and empowered according to an economic logic. Most of the managersin the reformed Norwegian hospitals are still managing in accordance with a professional, medical logic. The exception is nurses who hold new management positions. Nurses areadapting through more involvement in and commitment to the new managerial expectations, more so than the physicians who more or less are ignoring the managerial reform attempts. The third article (”Management between autonomy and transparency”) investigates how therecent Norwegian hospital reform can be seen as a vehicle to transform the mode of managinga hospital. The traditional collegiate form of management characterizing Norwegian hospitalshas been challenged by deliberate efforts to implement general management, including theprinciple of unity of command. A simultaneous development, however, has been efforts atincreasing transparency, giving the public better information about the costs and quality of thehospitals. This article sets out to show how an executive group of hospital managers dealt withthese new organizational constraints. In accordance with the reform ideology, the managersunanimously accepted performance unit accountability, unity of command, and the distinctionbetween professional and general management. However, they also pointed out dissonancebetween their assumed accountability and their lack of power. To resolve this paradox, they cleverly set out to exploit the financial incentive system to increase revenues. This was revealedby investigative reporters taking advantage of publicly accessible databases concerning theoperations performed in hospitals. The fourth article (”Hospital managers’ perceptions of the conditions of management inDenmark and Norway”) compares hospital managers’ perceptions of their managementcontext in Denmark and Norway. Survey data from recent Danish and Norwegian studies areused to compare and contrast the two countries particularly in regard to four themes: 1)perceptions of the hospital’s relation to its environment, 2) perceptions of staff motivation, 3)perceptions of stimuluses for change, and 4) value orientations. The main purposes of thearticle are, thus: 1) to investigate variation in perceptions of management conditions in twocountries that may appear very alike in cultural terms; 2) based on this, to investigate the impactof the Norwegian hospital management reform in 2002; and finally 3) to develop more generalstatements about hospital management based on data from the two countries.
