I den forbindelse besvares to hovedforskningsspørsmål.
ENGLISH SUMMARY:
The fundamental limitation in public values scholarship lies in its predominant focus on classifying and identifying what constitutes public values. As a result, the scholarship pays little attention to how and why public values—such as equity and human rights—gain importance at the global level and move across governance levels particularly from the global to national and local contexts in the Global south. This study addresses this gap by positioning the analysis of global public values within the context of intergovernmental organizations (IGOs) in global health governance. In doing so, it addresses two main research questions. First, how and why do interactions among diverse actors—and the mechanisms and processes they deploy—shape the movement of global public value for equity in health across levels in Tanzania? Secondly, to what extent do these movements produce institutional effects that reflect national and local realities?
Guided by organizational and institutional theory, which offers a multi-level analytical lens, the study employs a qualitative, multi-case design encompassing four empirical cases that connect the global, national, and local levels, with each empirical case addressing one aspect of the main research questions. The four empirical cases address specific questions on:(1) How does the World Health Organization (WHO) monitor compliance with soft laws without hierarchical enforcement power? (2) How and to what extent has equity in health, as articulated in WHO declarations, been institutionalized at the national level? (3) How does community participation, as an instrumental value for promoting equity in health articulated in WHO declarations, translate into local governance structures? (4) How do donors influence the implementation of public values through community participation in health planning in local government authorities? Data addressing these questions was collected through document analysis and semi-structured interviews with key informants at global, national, and local levels.
The findings from the four empirical cases contribute to answering the main questions by demonstrating different processes, mechanisms, and actors, and the way they shape
global public values as they move across governance levels. Analyzed through metaorganization, the empirical case, which addresses the specific research question on how the WHO monitors compliance with soft laws, shows that the organization relies on interdependent relationships across its meta-, administrative, and national levels to compensate for structural limitations, as member state sovereignty constrains the use of hierarchical enforcement mechanisms for monitoring. As such, the WHO uses monitoring mechanisms that combine collective authority, bureaucratic processes, and collaboration. Collectively, these mechanisms foster mutual adaptation and voluntary compliance, thereby facilitating the movement of global public values embedded in soft law to the national level. However, this case also demonstrates that compliance is mediated by power asymmetries whereby Tanzania's reliance on WHO for financial and technical assistance amplifies WHO's influence.
Building on the monitoring compliance case, the empirical case study on the institutionalization of global public value of equity in health addresses the second specific research question. Drawing on ‘’new’ and ‘old’ institutionalism, the findings show how both external mechanisms—such as external legitimacy and normative pressures —and internal factors —such as culture, historical legacies, and the national ideology of Ujamaa, which stresses on collectivism, shape the institutionalization outcome of global public value of equity in health in Tanzania. Thus, the institutionalization of global public values in Tanzania shows a mix of homogenizations, with some equity elements adopted exactly as in WHO declarations,
and selective adoption, where others were modified or excluded. This outcome promotes stability by embedding the core principles of equity in health into the Tanzania national policies and strategies, while allowing some elements to be adapted, which enhances their durability. Furthermore, the translation case examines how does community participation, as an instrumental value for promoting equity in health articulated in WHO declarations, translate into local governance structures? Using translation theory, it shows how community participation is transformed from a soft, instrumental global value into a formalized, structured process within local governance, which is then integrated into health planning. The findings further indicate that translation can lead to partial homogenization, as Tanzania adopted the normative principle of community participation as articulated in WHO declarations, yet generated a localized definition that clarified both the meaning of community participation and the composition of the community that were not explicitly defined in the WHO declarations. Thus, the translation case demonstrates that community participation, globally expressed in a soft, general form, can be hardened and shaped by internal and
external mechanisms such as aid conditionality. Thus, this case concludes that the translation of global public values when moved to the national and local level is a context-dependent process rather than a passive adoption. As such, national actors retain the agency to reinterpret and integrate these values into local governance structures.
The fourth empirical case analyzed through resource dependency theory illustrates that donors shape the practice of community participation through conditionalities, such as
performance-based approaches that tie funding to specific targets, indicators, and priorities. This mechanism reinforces hierarchical control, thereby enhancing compliance and promoting donor interests under the shadow of hierarchy. This dynamic reduces citizens to passive recipients, compromising the normative principle of equity and raising a critical question: equity for whom, and under whose terms? This case concludes that, in contexts of resource dependency, the institutional outcomes of global public values, when moved to the local level, often reflect external interests and priorities, resulting in compliance that is poorly aligned with local contexts. In general, the four empirical cases show that the movement of global public values across governance levels causes variations and shifts, not only in the processes, mechanisms, and institutional outcomes but also in the roles, influence, and interactions of the actors involved in shaping them. As such, the movement reveals asymmetrical power relationships, interdependencies and dependencies, the agency of national actors, hierarchies, and the influence of historical legacies, political ideologies, and culture, all of which shape the pathways through which global public values move across levels.
Therefore, the extent to which these movements create institutional effects that correspond to national and local realities is both varied and contingent. Following that, this study makes several contributions. The first contribution of this thesis lies in its ability to foster cross-disciplinary dialogue linking global public values and IGOs in health governance. As such, it responds to the scholarly calls to advance the field by moving beyond its currently ambiguous and unbounded conceptual status (Jørgensen & Bozeman, 2007; Van der Wal, Nabatchi, & de Graaf, 2015). This involves embracing a broader, more inclusive framework that recognizes the shifting, permeable boundaries of public value theory (Fukumoto & Bozeman, 2019) and incorporates diverse theoretical and disciplinary perspectives. By integrating an IGO perspective, the study illustrates that public values are not entirely national but are co-created through ongoing interactions and consultation between IGOs and states, challenging the conventional focus in public values studies on the state or national institutions as primary units of analysis. It further shows that public values are not only enacted through national governance instruments (Fukumoto & Bozeman, 2019; Wæraas, 2014; Rosenbloom, 2014; Baehler et al., 2014). Instead, they can also be expressed and embedded in non-binding instruments, such as soft law, and shape member states' behavior. This insight illuminates a limitation in traditional public values theory: its overemphasis on national governance instruments. It further highlights the critical, yet often overlooked, role of donors, IGOs, and international bureaucracies in public values governance. Integrating an organizational and institutional perspective offered a study with a multi-level analytical framework, contributing to explaining how and why public values are mutable as they move across levels of governance and the mechanisms that shape them. By grounding the empirical analysis in the Global south, this study contributes to addressing the geographical imbalance in public values scholarship (Jørgensen and Sørensen, 2012; Van der Wal, 2016). This engagement advances ongoing scholarly efforts to develop a more pluralistic universalism in public values (Bozeman, 2007; Van der Wal et al., 2015) by including ideas, and practices from non- Western contexts. Through this, the Tanzania case provides practical implications and highlights the inherent public values governance dilemmas faced by public administrators in a resource-dependent environment, who must reconcile global public values with domestic policy priorities while addressing the needs of the poor.
