Siri Håvås Haugland is a professor in psychosocial health, and has a masters degree in social work and a PhD in public health and general practice from the Norwegian University of science and technology (NTNU). Her PhD-thesis from 2014 was "HAZARDOUS ALCOHOL USE ACROSS GENERATIONS. Parental and offspring hazardous alcohol use in the Nord Trøndelag Health Study». Her research has since focused on adverse childhood experiences, with a particular focus on parental alcohol use) and harmfull outcomes among children.

Siri has worked within the drug and alcohol field since 1996, primarily at a regional resource center for drug and alchol research, treatment and prevention. She has also workexperience from project management, planning and clinical work with young substance users. In august 2014 the started working at the University of Agder as an associate professor at the Department of psycosocial health, and from january 2016 as the head of department.

Parental practices strongly influence offspring development, and parental alcohol use may affect parenting behavior. However, most studies have focused on child-related outcomes. This review instead examined associations between parental drinking and parenting practices. Following PRISMA guidelines, a systematic search in MEDLINE, PsycINFO, Embase, and Scopus identified 9053 articles. Of these, 222 full texts were screened by two reviewers, and 77 were included for critical appraisal. After quality assessment, 68 studies published between 1991 and 2026 were reviewed. Studies were included if they (i) measured parental alcohol use as a predictor and parenting practices as an outcome, (ii) involved offspring under 20 years, (iii) had a quantitative design, (iv) were peer-reviewed and published in English, and (v) excluded pregnancy measures and alcohol/parenting treatment interventions. Parenting factors (e.g., parent–child relationship, monitoring, communication, discipline, conflict, abuse) were categorized and grouped into general and alcohol-specific practices. Parental drinking was categorized into alcohol use disorders (AUD) and non-dependent alcohol use. Sixty-four studies reported at least one significant association between parental drinking and poorer parenting practices. Mixed results were observed for non-dependent drinking and alcohol-specific parenting. Some associations varied with parental gender. Overall, different drinking patterns appear to be linked to impaired parenting quality. Despite being the most common pattern of alcohol use, non-dependent drinking remains understudied in parenting research. The present review highlights several critical gaps in the literature, particularly regarding the relationship between non-dependent drinking, maternal AUD, and parenting practices. Moreover, contextual factors, such as socioeconomic status and gender differences, are insufficiently explored, limiting the understanding of heterogeneity in risk and outcomes. Future research would benefit from employing robust longitudinal designs and expanding geographical representation, in order to capture variation across sociocultural contexts and enhance the generalizability of findings.

Background: Substance use problems (SUP) have significant adverse consequences not only for individuals with such problems, but also for their loved ones. One important concern is exposure to violence among affected family members. This study investigates whether close family members, specifically partners or parents, of individuals with SUP, including alcohol, face increased risks of physical and sexual violence and emotional abuse within the last 5 years. We also examined a hypothesized mediation model linking SUP exposure, violence, and quality of life (QoL).Methods: We conducted a cross-sectional analysis using data from 17,640 adults aged 23 years and above who participated in the Norwegian Counties Public Health Survey 2023. Multivariable logistic regressions were performed, controlling for age and socioeconomic status, and stratified by gender. Latent regression analysis was used to examine the relationship between SUP exposure and QoL.Results: Gender-specific analyses revealed that partners or parents of individuals with SUP had significantly higher odds of experiencing physical violence (women OR 4.54; men OR 4.14), sexual violence (women OR 4.31; men OR 4.80), and emotional abuse (OR 2.96 across genders). These family members also reported lower QoL. The association between family member status and lower QoL was statistically accounted for by experiences of violence. Information on the perpetrator was not available, and the source of reported violence could therefore not be determined.Conclusion: Close family members such as partners or parents of individuals with SUP face substantial risks of violence and emotional harm and report lower QoL. Support services should offer gender-sensitive, tailored assistance that addresses family members' needs as individuals, not solely as caregivers. These findings highlight the importance of recognizing affected family members as a vulnerable population within public health initiatives.

Abstract Background Childhood adversities can have long-term impacts on quality of life (QoL), health, and well-being. We surveyed an adult population to assess the prevalence of three common adverse childhood experiences (ACEs): parental alcohol problems, parental mental illness, and violence from caregivers, as well as the overlap between them. We additionally examined the prevalence of adult support across ACE exposure and how this support related to perceived childhood difficulties and adult QoL. Methods This cross-sectional study used retrospective data from the Norwegian Counties Public Health Survey in Agder 2023. Data were summarized descriptively, and a Venn diagram illustrated the overlap between ACEs. We examined associations between cumulative exposure and the presence or lack of adult support and how these factors related to perceptions of childhood difficulties and adult QoL using one-way analysis of variance. Results The most prevalent ACEs were parental mental illness and parental alcohol problems (16% and 14%, respectively), while ~ 10% reported violence from a caregiver. We observed substantial overlap amongst the three ACE categories, with 2.4% having experienced all three. Adult support was inversely associated with higher levels of ACE exposure, and analysis showed a pattern of increasing perceived childhood difficulties and decreasing QoL scores. Adult support was consistently associated with the perception of a less difficult childhood and higher QoL, among subjects with the same level of ACE exposure. Conclusions Our findings emphasize the importance of adult support within the context of ACEs, and the need for enhanced measures aimed at supporting those most affected.

Background Child maltreatment (CM) is a significant public health concern with long-term consequences for the child, including an increased likelihood of engaging in risky health behaviors, such as substance use, as well as impaired quality of life (QoL). The relationship between CM and specific substance use patterns, particularly the use of nicotine products such as Swedish snus, remains understudied. We investigated the relationship between CM and the use of nicotine and alcohol, as well as whether use of these substances mediates the association between CM and adulthood QoL. Methods This study utilized data from the Norwegian Counties Public Health Survey for Agder in 2023. Latent regression analysis was employed to examine the association between CM and QoL, as well as potential mediated associations involving substance use, while controlling for sociodemographic factors. Results Nicotine use in the past year was significantly more prevalent among individuals with a history of CM compared to the group with no history of CM (35.8% vs. 25.6%, p < 0.001). Both smoking (24.3% vs. 15.6%, p < 0.001) and snus use (17.6% vs. 13.3%, p < 0.001) were more common in the CM group, with differences remaining significant after adjustment for sociodemographic factors. No significant differences were found in alcohol use across various measures, including frequency of use, AUDIT-C scores, and binge drinking. Latent regression analysis showed that CM had an overall negative association with QoL [β = −0.61; 95% confidence interval (CI) −0.71, −0.51; p < 0.001]. A significant mediated association involving nicotine use was also observed (β = −0.12; 95% CI −0.16, −0.07; p < 0.001). Conclusion CM is associated with a higher prevalence of smoking and snus use, which are negatively associated with QoL. The findings suggest that targeted efforts to support QoL among individuals with a history of CM, including attention to nicotine use, may be relevant within broader strategies addressing living conditions and psychological wellbeing.

Aims Children and adolescents living with parental substance use problems (PSUP) are at potential risk of developing cognitive, behavioral, psychosocial and emotional difficulties affecting their health. However, little is known about how they themselves understand and promote their own health and well-being. This meta-ethnography aims to integrate and synthesize qualitative studies that have explored – from a salutogenic health-promoting perspective – how adolescents living with PSUP express health promotion and well-being, focusing on resources for positive health. Methods The seven iterative phases by Noblit and Hare were followed. Systematic literature searches yielded 13 articles. Results “Designing their own story” was identified as an overarching metaphor accentuated by three main themes that we named: (1) “controlling before opening up”; (2) “choosing trusted support”; and (3) “learning for life”. The overarching metaphor symbolizes how adolescents living with PSUP demonstrate agency in a stressful life situation, expressing a sense of control in everyday life and actively choosing whom to involve for support. Conclusions Supplementing protection and prevention, a different view on narrative of adolescents living with PSUP emerges in the conceptual framework of salutary health promotion. When support is offered, this can inform professionals, service providers and policymakers about the significance of acknowledging and exploring the resources within the adolescents themselves and in their environments. Further research is needed to gain a broader and deeper understanding of how to support adolescents living with PSUP in a health-promoting direction, recognizing their potential to improve their health and well-being in their stressful and challenging life situation.

Background: Mental health problems and/or problematic substance use (including alcohol) affect not only the individuals directly involved but also their close family members. Being a concerned other, a partner, parent, or child of someone with mental health problems or problematic substance use can be emotionally demanding and may negatively impact sleep health. This study aimed to examine whether concerned others are at increased risk of sleep-related symptoms compared with individuals without such family-related burdens.Materials and methods: A cross-sectional study was conducted among 18,517 Norwegian adults (>18 years) by completing an online self-report questionnaire. Multivariable logistic regression models adjusted for age, economic capability (SES) and gender were used to examine whether being a partner or parent of individuals struggling with substance use or mental health problems or having experienced a parent with problematic alcohol use or mental health problems during childhood was associated with sleep-related symptoms.Results: During childhood, 15.8% and 13.9% reported having parents with mental health problems or problematic alcohol use, respectively. In adulthood, 1 in 6 individuals reported being a partner or parent to someone with mental health problems, and 1 in 10 reported being a partner or parent to someone with problematic substance use. Childhood exposure to parental mental health problems or problematic alcohol use was significantly associated with increased odds of sleep initiation difficulties, sleep maintenance problems, daytime sleepiness, and chronic sleep issues in adulthood (ORs ranging from 1.43 to 1.73). Similarly, being a concerned other to someone with mental health problems or problematic substance use in adulthood was associated with increased odds of having the same sleep-related symptoms (ORs ranging from 1.56 to 1.95).Conclusion: Concerned others of individuals with mental health problems or problematic alcohol or substance use had higher prevalence of sleep-related symptoms. Both current caregiving and childhood exposure to such experiences are linked to poor sleep and potential long-term health consequences. These findings highlight the need for targeted support and early interventions to improve sleep and overall well-being among concerned others.

Background Discrimination has adverse effects on people’s health and quality of life (QoL). This study aimed to identify distinct intersecting patterns of discrimination experiences, examine their associations with social categories and health factors, and assess the impact of perceived discrimination (PD) on health and QoL outcomes beyond specific self-reported reasons for PD. Methods We utilized data from the Norwegian Counties Public Health Survey (NCPHS) conducted in Agder County in 2023 and employed latent class analysis (LCA) to explore how patterns of discrimination reasons cluster. The selected classes were then further examined to determine how they differed by comparing each class to the reference class with no PD. Lastly, we assessed the estimated marginal means of each class on health and overall QoL using ANCOVA. Results The study identified six classes of PD: Massive PD, Gender/Age PD, No PD, Function/Illness PD, Ethnicity/Skin PD, and Political PD. ANCOVA analyses revealed significant differences across self-rated health, mental distress, and QoL. Notably, the Massive PD, Gender/Age PD, and Function/Illness PD groups reported significantly poorer self-rated health and QoL compared to the No PD group. All PD classes scored significantly higher in mental distress than the No PD group, with the Massive PD class exceeding the clinical cut-off, indicating elevated psychological distress. Conclusions Our findings reveal persistent health and QoL disparities between individuals experiencing PD and those who do not, despite a robust welfare system. Service providers must consider the interplay of factors such as age, gender, income, and health conditions with PD to ensure fair service delivery.

Across the world, men consume more alcohol and experience more alcohol-related harm from their own drinking than women. Men’s alcohol use also results in more harm to others, including harm to women and children. However, relatively few studies have focused on the scope and impacts that men’s harmful alcohol use may cause other people, and few studies or policies specifically address harms that women and children experience because of men’s alcohol use. Drawing on three recent reviews of the literature covering harm to women, harm to children, and policy options for reducing harmful drinking by men, we have synthesized the evidence to inform future alcohol, health, and social policy implementation; service and response system development; and directions for research. Findings suggest possible system changes to continue advancement toward health and well-being for women and children around the world.

Place attachment (PA) is recognised for its influence on health and well-being. We validated a brief PA measure called the PA-4, which focuses on the individual's place of residence and surrounding neighbourhood. The study included 17,487 participants from a national survey conducted in Norway in 2021. The PA-4 comprises four indicators scored on a 0–10 scale. Emotional attachment was measured through indicators of belongingness and feeling safe, while cognitive evaluation was assessed through indicators of satisfaction with housing and satisfaction with the place of residence. The analysis examined the factor structure, internal consistency, and discriminant validity of the measure using multi-group confirmatory factor analysis, comparing immigrants and native-born individuals. Convergent validity was assessed by examining the association between the PA-4 and a well-being scale through latent regression analysis. The PA-4 demonstrated good discriminant validity, as evidenced by a hypothesised difference in PA. The latent mean of the PA-4 was 0.65 (95% CI = 0.57–0.73, p < 001) lower for immigrants compared to natives. The regression analysis also supported convergent validity, as shown by PA-4's positive association with well-being. The PA-4 exhibited robust psychometric properties, indicating its suitability for use in multidisciplinary research.

Background Many people experience discrimination based on factors such as age, gender, illness, disability, ethnicity, skin color, religion, beliefs or sexuality. Discrimination can lead to marginalization and be linked to poor health. As a result, there is a growing interest in exploring the underlying causes of discrimination and its cumulative consequences. The aim of the present study was to explore the prevalence of perceived discrimination (PD) in a large cross-sectional public health study and its association with self-rated health and mental distress. Methods The study was based on data from 18,517 participants in the 2023 Norwegian Counties Public Health Survey (NCPHS) in Agder County. The participants were asked if they had experienced discrimination in the past 12 months and could specify different reasons. Furthermore, they were asked about their self-rated health, mental distress, and sociodemographic factors. In the analysis of the responses, PD was categorized based on the number of reasons, ranging from no PD to one, two, or three or more reasons. Results Among the participants, 17.8% reported experiencing discrimination for at least one reason. Compared with those who experienced no discrimination, participants reporting discrimination for one reason were twice as likely to report poor self-rated health or mental distress. The odds increased to a range between two and a half to three times for those facing discrimination on two grounds. For individuals experiencing discrimination for three or more reasons, the likelihood rose substantially: Four times higher for poor self-rated health and approximately three times higher for mental distress. Conclusions There is a clear association between multiple reasons for PD and low scores in mental and/or physical health, suggesting a cumulative influence. To address this issue effectively, prevention efforts should be implemented at both the national level and within local public health strategies.

Introduction: Parental alcohol problems may cause immediate and long-term harm in children. This study investigated the relationship between parental alcohol problems and adult social connection, and whether the absence of adult support during childhood compounded these relationships. Methods: We used cross-sectional data from 23,714 adults (> 18 years) who participated in the 2019 Norwegian Counties Public Health Survey. Parental alcohol problems were assessed using CAST-6. Social connection was measured using objective (participation in social activities) and subjective indicators (missing someone to be with, feeling excluded, isolated or lack of support). Multivariable logistic regression analysis was used to analyse associations between parental alcohol problems (with or without adult support) and adult social connection, adjusted for sex, age and socioeconomic status. Results: Individuals reporting parental alcohol problems had higher odds of poor social connection across all indicators compared to those without these experiences (odds ratios [OR] and 95% confidence intervals [CI] ranged from OR 1.14; CI 1.01–1.29 to OR 1.49; CI 1.26–1.76). These associations were notably stronger for those who also lacked adult support during childhood, particularly in subjective indicators of social connection, with OR ranging from 1.50; CI 1.36–1.67 to OR 4.02; CI 3.49–4.64. Discussion and Conclusions: Findings revealed associations between parental alcohol problems and lack of social connection in adulthood. The negative impact was amplified when adult support during childhood was lacking, particularly affecting the subjective measures of social connection. These findings highlight the long-term consequences of parental alcohol problems and underscore the potential protective role of adult support.

Vanskelige barndomsopplevelser kan sette negative spor i folks helse helt inn i voksen alder. Dette har fått stor oppmerksomhet i forskningslitteraturen de siste 20 årene (Bellis et al., 2019; Hughes et al., 2017). I dette kapittelet ønsker vi å se om internasjonale funn fra dette forskningsfeltet bekreftes i en norsk folkehelseundersøkelse. Spesifikt vil vi undersøke i hvilken grad vanskelig-heter i barndommen er relatert til psykososiale konsekvenser (sosiale forhold, økonomisk situasjon og psykisk helse) i voksen alder.Internasjonalt blir temaet omtalt som «Adverse Childhood Experiences» (ACE), etter navnet på en større amerikansk studie om fenomenet fra midten av 90-tallet (Felitti, 2019). Bakgrunnen for studien var at klinikere i en helse-forebyggende poliklinikk oppdaget hvor vanskelig det var for pasienter i et behandlingsprogram for fedme å gjøre livsstilsendringer for å gå ned i vekt og å holde vekten nede (Felitti et al., 2010; Olsen, 2020). Når de utforsket dette nærmere, oppdaget de at spisevanene kunne være en måte å håndtere under-liggende emosjonelle problemer forårsaket av seksuelle overgrep i barndom-men. Oppdagelsen førte til at de begynte å spørre alle pasienter med fedme om barndomstraumer, og de ble overveldet over hvor mange som hadde slike negative barndomsopplevelser. I en pilotstudie oppdaget de at så mange som 55 % av pasientene hadde opplevd seksuelle overgrep i barndommen (Felitti, 2019). Forskerne ble rådet til å gjøre en større epidemiologisk studie for å undersøke forekomsten blant sykehuspasienter generelt, og ACE-studien ble igangsatt i 1995 blant medlemmer i et større privat sykehusforetak i USA, Kaiser Permanente i San Diego. Internasjonalt har vanskelige barndoms-opplevelser blitt målt på ulike måter, alt fra enkeltspørsmål til større spørre-skjemaer – men mest kjent er nok fremdeles de ti spørsmålene fra ACE-studie som blant annet inneholdt spørsmål om emosjonelt eller seksuelt misbruk, omsorgssvikt, foreldres rusbruk og skilsmisse.

Background Globally, outpatient programs for substance use disorder (SUD) treatment have gained prominence. To assess the broader clinical implications of this trend we investigated shifts in functioning experienced by outpatients undergoing treatment. Methods We describe the clinical characteristics of a cohort of 93 SUD patients in a Norwegian outpatient treatment clinic. Using paired-samples t-tests, we examined changes in perceived functioning, mental distress, and other clinically relevant outcome variables in a 5-month time interval during the treatment course. Results We obtained follow-up data for 67 (72%) of the included patients, with no significant difference in patient-related factors between those who completed the treatment course and those who were not assessed at follow-up. Perceived functioning increased significantly from study inclusion (Time 0) (mean 19.8, standard deviation ± 8.8) to its conclusion (Time 1) (24.3, ±9.3; t (66) = 4.5, (95% CI: 2.5–6.5, p < 0.001). We also identified significant improvement in most other measured variables, including mental distress, self-reported sleep quality, restlessness, and obsessive thinking. Substance use–related variables showed a modest, non-significant improvement at T1. Conclusion During a 5-month course of outpatient treatment, patients’ subjective experience of functioning improved significantly. Those with the lowest functioning levels at T0 improved the most. Structured monitoring may be a valuable clinical tool for personalizing intervention, enhancing treatment outcomes, and supporting the clinical decision-making process.

Introduction Quality of life (QoL) assessment is essential in health-related research and clinical settings, offering insights into individuals’ well-being and functioning. This study validated the Essential QoL-3 (EQoL-3), an ultra-short scale assessing essential dimensions of QoL, for use in epidemiological research and clinical settings. Methods Data from a 2021 national survey in Norway (N = 17,487) were used. Three items on the EQoL-3 assess life satisfaction, happiness, and meaningfulness on a 0–10 scale. Discriminant validity was assessed by comparing the EQoL-3 with the Satisfaction with Life Scale (SWLS) and by examining latent mean differences between individuals with adverse life experiences (ALE+) and those without such experiences (ALE-). Convergent validity was evaluated through latent regression analyses comparing the EQoL-3 with a perceived mastery scale and a mental distress scale. Results The discriminant validity of the EQoL-3 was less than optimal when compared with the SWLS. Background There is increasing need for prospective investigations in the preventing role of health-related behaviours on mental health problems. The aim of this study is to identify patterns of health-related behaviours in adolescence, and the association between the behavioural patterns and the subsequent diagnoses and/or drug treatment for anxiety and/or depression in adulthood. Methods This prospective study consisted of 13–19-year-old participants in the Trøndelag Health Study (Young-HUNT3) in 2006–2008 (n = 2061, 1205 females and 856 males) in Norway, who also participated in HUNT4 (2017–2019). Survey data on health-related behaviours in adolescence, including low level of physical activity, low consumption of wholegrain bread, fish, fruit, vegetables and high consumption of sugar-sweetened beverages and insomnia were linked on an individual level to prospective information on drug use and diagnosis in national health registries. The different patterns of health-related behaviours were identified through latent class analysis. Subsequent anxiety or depression was defined as at least one recording in either of three registries covering recorded diagnosis in primary and specialist healthcare, or dispensed prescription drugs during 2008–2019. Additionally, self-reported psychological distress measured in young adulthood was applied as a supplemental outcome measure. Results Four patterns of health-related behaviours were identified: high risk behaviours (class 1), moderate to high risk behaviours (class 2), low to moderate risk behaviours (class 3) and low risk behaviours (class 4). Adolescents in class 3 showed higher odds of subsequent diagnoses for anxiety and/or depression in primary and specialist healthcare compared to class 4 participants. In addition, both class 1 and class 4 participants had higher odds for self-reported psychological distress than those class 4 (OR = 1.56 and OR = 1.86, respectively). Conclusions Our findings suggest that health-related behaviours are clustered among Norwegian adolescents. The patterns of unhealthy behaviours during adolescence only partly increased the risk of anxiety and depression in adulthood. Promoting healthy behaviours during adolescence may potentially reduce the burden of mental illness in adulthood, but further research is needed to clarify the nature of the relationships.

Background Infralow neurofeedback (ILF-NF) was recently developed as a subtype of traditional, frequency-based neurofeedback that targets cerebral rhythmic activity below 0.5 Hz and improves brain self-regulation. The efficacy of ILF-NF in the treatment of substance use disorder has not yet been evaluated, but clinical evidence suggests that it may prevent relapse by improving functioning in various life domains. The current study aimed to fill this research gap and extend empirical evidence related to this issue. Methods Ninety-three patients with substance use disorders at an outpatient unit in Norway were randomized to receive 20 sessions (30 minutes each) of ILF-NF training combined with treatment as usual (TAU), or TAU alone. The primary outcome was quality of life post-treatment as an overall measure of functioning. We analyzed between-group differences using Student t tests. Results We found no significant differences in quality of life between groups. We found similar nonsignificant results for most of the secondary outcome measures, including drug use, sleep, anxiety and depression. Compared to TAU, the ILF-NF + TAU group reported significantly lower restlessness scores post-treatment (mean difference −1.8, 95 % confidence interval −3.1 to −0.5; p = 0.006). Limitations: This study was limited by broad inclusion criteria and a lack of placebo control (sham neurofeedback treatment). Conclusion ILF-NF offered limited additional benefit when combined with TAU, except in the area of restlessness. Future studies could further investigate the relationship between ILF-NF, restlessness and substance use in targeted subpopulations to illuminate relapse mechanisms.

The aim of the present study was to examine the associations between self-reported childhood difficulties, weight status, and lifestyle behaviors among a representative sample of Norwegian adults. This cross-sectional study included 28,047 adults (>18 years old) living in southern Norway. A self-report questionnaire was used to assess information about the overall quality of the respondents’ childhood retrospectively in addition to current weight status and current lifestyle behaviors. Multivariable logistic regression models adjusted for gender, age, and educational level showed that evaluating childhood as difficult was associated with increased odds of obesity (OR: 1.29; 95% CI; 1.16–1.44) in adulthood. Moreover, a difficult childhood was associated with increased odds of unhealthy lifestyle behaviors in adulthood, including low consumption of fruit and berries (1.21; 1.09–1.34) and fish (1.43; 1.30–1.57), high consumption of sugar-sweetened beverages (1.30; 1.14–1.48), low level of physical activity (1.10; 1.01–1.21), smoking cigarettes (1.78; 1.61–1.97), and using smokeless tobacco (1.20; 1.07–1.36). Overall, results from the present study suggest that experiencing childhood as difficult is associated with an increased risk of obesity and a range of unhealthy lifestyle behaviors in adulthood. Thus, our findings highlight the importance of identifying and providing support to children in difficult life circumstances in addition to customized and targeted public health efforts in adulthood.

Background: There is an international public health interest in sustainable environments that promote human wellbeing. An individual’s bond to places, understood as place attachment (PA), is an important factor for quality of life (QoL). The material environment, such as access to nature (AtN), access to amenities (AtA), or noise, and the social environment, such as social support or loneliness, has the potential to infuence PA. The aim of the present study was to explore the relationship between these factors and QoL. Methods: The study relied on data from 28,047 adults from 30 municipalities in Southern Norway obtained from the Norwegian Counties Public Health Surveys in 2019. Latent regression analyses were used to examine the relationship between the material and social environmental factors and QoL, mediated by PA. Results: We found a relationship between social and material environmental factors and PA. Higher AtN and AtA scores were related to an increase in PA, whereas higher perception of noise problems was related to decreased PA scores. When social environment factors were added to the model, they were even stronger predictors of PA and, in turn, QoL through mediated efects. We also found a strong positive association between PA and QoL (unstandard‑ ized β=0.88, 95% CI=0.87–0.90, p<0.001). The whole model explained 83% of the variance in PA and 65% of the variance in QoL. Conclusions: Taken together, the fndings suggest the relevance of material and social environmental factors for PA and QoL. Therefore, research on public health and QoL should include place-sensitive variables.

Objectives: This study examined associations of three prevalent adverse childhood experiences (ACEs) - parents' problematic alcohol use (PPAU), parental separation/divorce, and lack of support from a trusted adult-with adult quality of life (QoL), and potential mediators of associations. Methods: Data were from a representative survey (N = 28,047) in Norway that assessed ACEs, QoL, and potential mediators: enduring perceptions of childhood hardships (Difficult Childhood Questionnaire; DCQ), current mental distress, and current social isolation. Latent regression analyses examined potential mediators. Results: Each ACE was positively associated with perceptions of childhood as difficult (higher DCQ scores). In turn, ACEs were negatively associated with adult QoL through indirect effects. Lack of support from a trusted adult had the strongest negative association with adult QoL, compared to PPAU and parental separation/divorce. The association between the ACEs and QoL was explained through the mediators of mental distress and social isolation. Conclusions: Of the examined ACEs, lack of support from a trusted adult had the strongest negative impact on adult QoL. Adult support to vulnerable children could potentially ameliorate adult consequences of ACEs. In addition, adults reporting difficulties due to childhood adversities may benefit from therapeutic interventions that address both psychological distress and isolation.

Mestring er et etablert begrep i arbeidet med å forebygge psykisk uhelse for ungdom. I 2020 kom mestring inn i læreplanen for grunnskolen gjennom det tverrfaglige temaet folkehelse og livsmestring. For å forstå mestring blant ungdom er konteksten og sammenhengen hvor handlingene foregår viktige momenter. I denne studien utforsker vi hva jenter selv gjør for å mestre sosiale utfordringer i skolen. Gjennom en innholdsanalyse av totalt 22 intervjuer med ti jenter om deres hverdag, og ved hjelp av begrepet arbeid, finner vi at jentenes mestringsarbeid er mangfoldig og har ulik intensjon. Unngåelse er et mestringsarbeid som er særlig utbredt. Vi forstår dette som et viktig mestringsarbeid for jentene, og drøfter hvordan våre funn kan utfordre gjengs forståelse av unngåelse. Studien inviterer dermed til en bredere forståelse av hvordan ungdom selv mestrer hverdagen sin, og hva som kan være formålstjenlig mestringsarbeid.

Background Appointment no-show and early dropout from treatment represent major challenges in outpatient substance use disorder treatment, adversely affecting clinical outcomes and health care productivity. In this quasi-experimental study, we examined how a brief reminder intervention for new patients before their first appointment affected treatment participation and retention. No-shows (not attending any sessions) and dropouts (discontinuation of initiated treatment because of three consecutively missed appointments) were compared between a period with pre-admission telephone calls (intervention) and a period without such reminders (non-intervention). Methods Participants were all eligible patients (N = 262) admitted to a Norwegian specialist clinic for substance use disorder treatment. We used the Chi-square test for the no-show analysis. Of the eligible patients, 147 were included in a subsequent dropout analysis. We used the number of visits up to 10 appointments as a measure for time to event. Group differences were analyzed using a Kaplan–Meier plot and the log-rank test. To control for relevant sociodemographic variables, as well as substance use and mental distress severity, we used Cox regression. Results No-show rates did not differ between the two periods (12% for non-intervention vs. 14% for intervention; χ2 = 0.20, p = 0.653). Of those consenting to participate in the dropout analysis (n = 147), 28 (19%) discontinued treatment within the time frame of 10 appointments, with no differences between the two periods (log-rank test = 0.328, p = 0.567). Controlling for baseline characteristics did not alter this finding. In fact, of the registered covariates at baseline, only higher education level was associated with attrition, linked to a reduced risk for dropout (hazard ratio = 0.85, 95% CI = 0.74–0.98, p = 0.025). Conclusion These findings do not provide support for the systematic use of a brief pre-admission telephone reminder in the current treatment setting. Background: This study’s purpose was to examine the association between a broad range of lifestyle habits and depressive symptoms in Norwegian adolescents. Methods: This study was based on national, self-reported, cross-sectional data from the Ungdata Surveys, conducted in 2017–2019. The target group comprised 244,250 adolescents (ages 13–19). Binominal logistic regression was used to analyse the association between lifestyle habits (physical activity, social media use, gaming, dietary habits, smoking, smokeless tobacco, alcohol intoxication) and depressive symptoms. The outcome measure was defined as a high level of depressive symptoms (≥80th percentile). Separate analyses were performed for boys and girls, and all models were adjusted for perceived family economy, parental higher education and age. Results: The odds of having depressive symptoms were significantly lower among those who reported being physically active at least 3 times per week (OR; boys: 0.81, girls: 0.83), used social media ≤3 h per day (OR; boys: 0.65, girls: 0.70), engaged in gaming ≤3 h per day (OR; boys: 0.72, girls: 0.77), were non-smokers (OR; boys: 0.74, girls: 0.72) and had not experienced alcohol intoxication during the previous 12 months (OR; boys: 0.66, girls: 0.67). Furthermore, the results indicated a significant inverse association between depressive symptoms and high consumption of a range of healthy food items and low consumption of unhealthy food and beverages among girls. Similar tendencies were found among boys (OR; 0.77–0.91). Finally, higher adherence to healthy lifestyle habits was associated significantly with lower odds of having depressive symptoms among both genders (OR; boys: 0.40, girls: 0.52). Conclusions: A healthier lifestyle was associated with lower odds of having depressive symptoms. Additional research is needed to confirm a possible causal relationship.

Aim: The purpose of this study was to estimate the prevalence of adverse childhood experiences (ACEs) among Norwegian adults from a general population and to identify potential associations with demographic and socioeconomic characteristics. Methods: A randomly drawn sample (N = 61,611) from the public registry of inhabitants was invited to participate in the Norwegian Counties Public Health Survey. The present study was based on online responses from 28,047 adults ≥18 years (mean age: 46.9 years, SD = 16.03). Log-link binomial regression analyses were performed to examine associations between four measures of ACEs (family conflict, lack of adult support, bad memories, and difficult childhood) and demographic (age, gender, civil status, parental divorce) and socioeconomic characteristics (education level, perceived financial situation, and welfare benefits). Results: Single individuals and those with parents that divorced during childhood were at elevated risk of all four ACEs. The risk varied to some degree between the sexes. The prevalence of ACEs declined with increasing age. We found a consistent social gradient that corresponded to the frequency of ACEs for all three socioeconomic characteristics investigated. The risks were highest for those in the lowest socioeconomic levels (RR: 1.53, 95% CI: 1.32–1.78 to RR: 4.95, CI: 4.27–5.74). Conclusions: Public health strategies should direct more attention to the interplay between ACEs and socioeconomic factors. Welfare services should be sensitive to ACEs among their service recipients.

Stress blant jenter ser ut til å være knyttet til press om å gjøre det bra på skolen og er videre relatert til psykiske plager. I denne artikkelen undersøker vi hvordan skolestress arter seg i unge jenters hverdagsliv, og hvilke institusjonelle prosesser jentenes erfaringer inngår i. Vi tar utgangspunkt i 22 intervjuer med 10 jenter på 10. trinn fra skoler i Norge og beskriver vurderingsarbeid – deres tankearbeid og gjøremål som er knyttet til testing, vurdering og individuell forbedring. Vi drøfter skolestresset og vurderingsarbeidet elevene gjør opp mot prinsippene om tydelig vurderingskultur og forbedring i skolen slik som det kommer fram gjennom den pedagogiske satsingen Vurdering for læring. Vi finner at styringsrasjonalitet i skolen kan bære i seg en invitasjon til arbeidsprosesser som kan få uheldige utfall for noen elever. Avslutningsvis trekker vi fram at skolestress ikke kun må forstås som et individuelt symptom, men også som et logisk tilsvar til styringsrasjonalitet i skolen. Dersom man vil forebygge skolestress, kan endringer i skolehverdagens innhold og krav være vel så hensiktsmessige som individuelle mestringstiltak.

Background Adverse childhood experiences (ACE) are related to adverse physical and mental health outcomes. However, few larger studies based on a general population sample with age groups ranging from young adults to elderly have investigated whether parental alcohol problems increase the risk of offspring subjective reports of ACE both during childhood and current adult adversities. The purpose of this study was to examine the associations between parental alcohol problems and adversities during childhood and later in adulthood. Methods The 28,047 respondents were adults (> 18 years old) from the general population who participated in the Norwegian Counties Public Health Survey. The study had a cross-sectional design and included respondents’ evaluations of childhood experiences and current adult adversities. The short version of the Children of Alcoholics Screening Test (CAST-6, cut-off ≥3) measured parental alcohol problems. Multivariable logistic regression was adjusted for gender, age, and education. Results Growing up with parental alcohol problems strongly increased the risk of experiencing a dysfunctional family environment during childhood (odds ratio [OR] 6.84; 95% confidence interval [CI] 6.36–7.36), perceiving childhood as difficult (OR 5.01; 95% CI 4.58–5.49), and reporting a lack of support from a trusted adult (OR 3.07; 95% CI 2.86–3.29). Parental alcohol problems were associated with a modestly increased risk of harmful alcohol use (OR 1.38; 95% CI 1.29–1.48), but the association with struggling with bad memories was strong (OR 4.56; 95% CI 4.17–4.98). Conclusions Parental alcohol problems increased the risk of offspring experiencing adversities during both childhood and adulthood. Providing supportive services to these children and their families and addressing this issue as part of treatment is important to prevent alcohol related harm.

This study aimed to investigate whether experience of parental alcohol intoxication was associated with adverse health outcomes among adolescents four years later. A population-based Norwegian cohort study of 2399 adolescents who participated in the Young-HUNT1 Survey 1995–97 (T1, 13–15 years old) was followed up four years later (T2) in 2000 (Young-HUNT2, 17–19 years old). Measures were based on adolescent self-report of exposure to parental alcohol intoxication, self-rated general health, mental distress (SCL-5) and lifetime hospital admission. Multivariable logistic regression analyses, adjusted for gender, age and parental education were applied. Results show that half of the adolescents (51%) had seen parents intoxicated at age 13–15 years. Four years later, those who had reported parental alcohol intoxication at T1 had increased odds of admission to hospital with odds ratios ranging from OR1.3; CI 1.0–1.7 to OR 2.2; CI 1.3–3.9, poorer self-rated health (odds ratio ranging from 1.8;1.2–2.6 to 2.0;1.1–3.7) and more mental distress (odds ratio ranging from 1.7;1.1–2.5 to 1.9;1.0–3.6). Furthermore, the increased frequency of experience of parental alcohol intoxication are associated with higher prevalence of admission to hospital and mental distress and lower levels of self-rated health. Findings from this large, representative population of Norwegian adolescents indicate that adverse health outcomes among adolescents in a general population are related to relatively common heavy drinking behaviours among parents.

Objectives: An increasing proportion of young Norwegians are categorised as too ill to attend upper secondary education, and poor physical and mental health may reduce their opportunities to return to school or find paid employment. This study examined the differences in self-perceived health, mental health, and prevalence of pain between Norwegian adolescents, who are not in education, employment or training (NEET) compared to adolescents who attend upper secondary school (age 16-21 years). Design: Cross-sectional study. Participants and setting: A total of 96 NEET youth and 384 age and gender matched adolescents attending upper secondary school in the south of Norway participated in the study. Main outcome measures: Self-perceived health, mental health and pain. Results: Multivariable analyses, adjusted for parental education, showed that more NEET girls reported poor self-perceived health (odds ratio 3.2; 95% CI 1.4–7.5) and poor mental health (2.4;1.0–5.2) when compared to girls who were attending school. The results showed no difference in the prevalence of various types of pain between girls who were attending or not attending school, and among boys the results showed no differences in health problems assessed in this study. Conclusion: The study indicates that NEET girls have poorer self-perceived health and poorer mental health when compared to girls who are attending upper secondary school. It will be essential to identify the causes of these health problems. This may provide a basis for specially adapted measures that could help more people in the target group return to school or paid employment.

Background: To examine changes in men‘s and women’s drinking in Norway over a 20-year period, in order to learn whether such changes have led to gender convergence in alcohol drinking. Methods: Repeated cross-sectional studies (in 1984–86, 1995–97, and 2006–08) of a large general population living in a geographically defined area (county) in Norway. Information about alcohol drinking is based on self-report questionnaires. Not all measures were assessed in all three surveys. Results: Adult alcohol drinking patterns have changed markedly over a 20-year period. Abstaining has become rarer while consumption and rates of recent drinking and problematic drinking have increased. Most changes were in the same direction for men and women, but women have moved towards men’s drinking patterns in abstaining, recent drinking, problematic drinking and consumption. Intoxication (among recent drinkers) has decreased in both genders, but more in men than in women. The declines in gender differences, however, were age-specific and varied depending on which drinking behavior and which beverage was taken into account. Conclusions: There has been a gender convergence in most drinking behaviours, including lifetime history of problem drinking, over the past 2–3 decades in this Norwegian general population, but the reasons for this convergence appear to be complex.

Background: Parental alcohol misuse may negatively affect drinking behaviours among offspring, but it is unclear to what extent influences are gender-specific and dependent upon the actual drinking behaviour measured. The aim of this study was to investigate whether hazardous drinking among Norwegian teenage boys (N = 2538) and girls (N = 2494) was associated with paternal and maternal alcohol misuse (CAGE). Methods: Definitions of hazardous drinking among offspring were based on self-reported alcohol consumption (in litres a year), frequency of drinking, and frequency of drunkenness. Based on this information, two composite measures of hazardous drinking were also constructed. Cross-sectional data from the Norwegian Young-HUNT 3 survey (2006–2008) were linked to information from biological parents who participated in the adult part of the HUNT study. Results: Logistic regression analyses showed that both boys and girls with alcohol misusing fathers were more likely to report high levels of alcohol intake compared to others of the same age and gender. This was contrary to boys with misusing mothers, who reported less alcohol consumption than other boys. Among girls, but not boys, high frequency of drunkenness was associated with maternal as well as paternal misuse. Conclusions: This study suggests that adolescent hazardous drinking is more prevalent among boys and girls with alcohol misusing parents versus those whose parents do not misuse alcohol. However, findings were gender specific and varied depending on the drinking outcomes under investigation. More evidence-based knowledge in this field is of great importance for better understanding the possible role paternal and maternal alcohol misuse may play in the development of hazardous alcohol drinking patterns among adolescent boys and girls.

Bakgrunn. Ungdommens alkoholbruk henger sammen med foreldrenes alkoholforbruk, men en eventuell sammenheng med beruselse hos foreldrene er lite undersøkt. Formålet med studien var å undersøke om det å ha sett sine foreldre beruset er assosiert med bruk av alkohol og eksperimentering med narkotika i tenårene. Materiale og metode. Studien er prospektiv og basert på data fra 2 399 tenåringer som deltok i Ung-HUNT 1- studien i 1995 – 97 og i Ung-HUNT 2- studien i 2000 – 01. Selvrapporterte spørreskjemadata og analyser ved bruk av logistisk regresjon, stratifisert på kjønn, ble benyttet. Resultater. Det å ha vært full mer enn ti ganger var for gutter assosiert med å ha sett foreldrene beruset (OR 3,7; 95 % KI 2,7 – 5,1), det samme gjaldt for jenter (OR 2,0; 1,5 – 2,6). Å drikke alkohol ukentlig eller oftere var assosiert med foreldreberuselse hos gutter (OR 2,2; 1,6 – 3,0), men ikke hos jenter – med mindre de hadde sett foreldrene fulle mange ganger (OR 2,4; 1,1 – 5,2). Å ha prøvd narkotika var assosiert med beruselse hos foreldrene både for gutter (OR 2,6; 1,7 – 3,9) og jenter (OR 1,6; 1,1 – 2,2). Fortolkning. Gjentatt beruselse, hyppig bruk av alkohol og det å ha prøvd narkotika var for tenåringer assosiert med det å ha sett foreldrene beruset. Det finnes andre forklaringsfaktorer studien ikke har kunnet kontrollere for, og ved tolking av resultatene bør det tas hensyn til det.

Psychosocial health among girls implies that girls’ perception of themselves is related to both their close surroundings and their more distant surroundings. Selfreported mental health problems among adolescents have increased and this is specifically evident for girls. Challenges related to mental health have received great emphasis throughout recent years in research, media and preventive mental health work. Regarding these issues, it is relevant to seek a contextualized understanding that explores the psychosocial health connected to everyday life and to broader societal structures among young girls.

Background Adolescent alcohol use is prevalent and associated with an increased risk of adverse consequences both in terms of acute injury and later and long-term problems. Adolescents with parents that misuse alcohol seem to be more vulnerable to developing hazardous drinking patterns. Many studies on parental alcohol misusers and their offspring have been based on clinical samples, and findings regarding possible gender effects on the relationship between parental and offspring alcohol use have shown inconclusive results. Heavy episodic drinking among parents is far more prevalent than parental alcohol misuse, but fewer studies have investigated the relationship between these kinds of hazardous drinking patterns among parents and offspring. Factors from area of residence may also influence the development of adolescent drinking patterns. Some studies have demonstrated that area-level adult alcohol use and socioeconomic status influence adolescent alcohol use, but few studies have had the possibility to investigate adolescent hazardous drinking pattern within the context of both parental drinking practices and area factors. Main aims This thesis mainly aimed to investigate whether, and to what extent, adolescent hazardous alcohol use in the general population was associated with parental hazardous alcohol use, and whether the possible coincidence of hazardous alcohol use was gender specific. An additional aim was to investigate whether adolescent hazardous alcohol use was associated with area-level factors when parental hazardous alcohol use and other factors on individual- and family-levels were taken into account. Material and method All inhabitants in the county of Nord-Trøndelag aged 13 years or older were invited to participate in the adult or the adolescent part of the Nord-Trøndelag Health Survey (HUNT and Young-HUNT). The studies in this thesis applied data from 1) adolescent participants from Young-HUNT1 and 2 (N = 2399), 2) adolescent participants from Young-HUNT3 that could be linked to data from biological mother and/or father participating in HUNT3 (N = 5032), 3) adolescent participants from Young-HUNT3 that could be linked to data from both biological mother and father participating in HUNT3 (N = 2306). Adolescent hazardous drinking measures were based on measures of frequency of drinking, frequency of heavy episodic drinking/intoxication, and alcohol consumption in terms of volume. Parental alcohol misuse and heavy episodic drinking were applied as measures of parental hazardous alcohol use. Measures of area factors were based on aggregated data from HUNT 3 (adult HED) and data from Statistics Norway (proportion of inhabitants with higher education). Multivariate analyses were performed by using logistic regression and generalized estimating equation (GEE), and results were presented as odds ratios with 95% confidence intervals. Main findings Adolescent hazardous alcohol use was associated with both alcohol misuse and heavy episodic drinking among parents. However, results varied depending on gender and the actual drinking behaviors investigated. Hazardous drinking among both boys and girls was associated with paternal alcohol misuse and HED. While hazardous drinking among girls was associated with both maternal misuse and maternal HED, this was not found among boys. On the contrary, findings revealed evidence of an aversive transmission regarding the associations between maternal alcohol misuse and high alcohol consumption among boys. Living in an area with a higher level of adult HED increased the risk of HED among girls, and this applied even when parental HED was included in the model. Results also showed a protective effect of living in the areas with higher SES, and this applied to both boys and girls. Conclusions Adolescent hazardous alcohol use was associated with both alcohol misuse and heavy episodic drinking among parents. Associations varied to some extent by type of/measures of drinking behaviors investigated, and by parental and offspring gender. Adolescent hazardous alcohol use was associated with area SES and area HED even though parental hazardous drinking was taken into account.
