  Background The transition from living at home to independent university life has been associated with deteriorated health outcomes, and many university students lack cooking skills. Life skills education promotes skills essential to mastering everyday challenges such as planning and preparing healthy meals. We developed a dietary life skills course to foster cooking skills and a healthy diet among university students. This paper describes the feasibility testing of this Skills for Life course.

  Background Public health challenges, ranging from noncommunicable disease prevention to pandemic preparedness, involve both policymaking and the handling of complexity at local, national or global level. The complexity of public health challenges arises from uncertain knowledge, hidden mechanisms, differing conceptual models and multiple stakeholders, making it difficult to identify and agree on problem definitions and potential solutions. This case study aims to provide insights into how complex public health challenges are addressed within local government agendas. We use theories of policy entrepreneurs and agenda setting to explore the processes behind the agenda setting of a public health programme for work inclusion. Methods We conducted an in-depth case study in a local government context. Data from interviews with key informants and political documents were triangulated and analysed using deductive thematic content analysis, framed within Kingdon’s agenda-setting theory and theories of policy entrepreneurs. Results We identified problem framing, the involvement of multiple actors, and turbulent surroundings as influential factors for agenda setting of complex public health challenges in local government. We further identified the significance of the level of influence of policy entrepreneurs’ positions, their coalitions and maintaining agenda control as core mechanisms for enabling agenda setting. We found that strategic manoeuvres of policy entrepreneurs were crucial to handle and manoeuvre complexity within the multiple streams described by Kingdon. Conclusions This article contributes knowledge of how complex public health problems receive attention and how these issues are shaped and defined, placed on governmental agendas, and paired with solutions. The article contributes important insights for understanding policy responses to complex public health problems and how policy entrepreneurs strategically manoeuvre within the streams of policy development to create windows of opportunity and maintain agenda control. This is important knowledge to support agenda setting for public health challenges and to realize policies to improve public health.

  Background: Few effective health interventions transition from smaller efficacy or effectiveness studies to real-world implementation at scale, representing a gap between evidence and practice. Recognising this, we have developed Nutrition Now – a tailored digital resource building on four efficacious dietary interventions, aiming to improve nutrition in the important first 1,000 days of life. Nutrition Now targets and guides expectant parents and parents of 0–2 year olds, serves as a reliable source of evidence-based information for midwives and public health nurses at maternal and child healthcare (MCH) centres, and offers pedagogical tools for early childhood education and care (ECEC) staff. The aim of this study is to implement Nutrition Now at scale and evaluate the impact of different sets of multifaceted implementation strategies on implementation outcomes. Methods: A quasi-experimental design with three study arms will be used, providing either low, medium or high implementation support, when rolled out in 50 municipalities in 2 counties in Norway. Nutrition Now will be implemented in MCH and ECEC settings and made available to expectant parents and parents of 0–2 year olds through social media and MCH. The implementation support builds on strategies described in the Expert Recommendations for Implementing Change (ERIC) implementation framework and is informed by dialogues with stakeholders. Impact of the different degree of implementation support will be assessed by examining reach, adoption, fidelity, and sustainability using usage data generated from the Nutrition Now resource, publicly available municipal data and qualitative interviews with MCH and ECEC staff. Discussion: Nutrition Now Phase 2 will break new ground by scaling up successively delivered and complementary dietary interventions in the first 1,000 days of life in a real-life context. The project also seeks to identify what level of implementation support is most effective when implementing digital, scalable, evidence-based early-life nutrition interventions in community settings. The project will inform implementation research and provide knowledge about effective implementation strategies to be used in a national scale-up of Nutrition Now.   This chapter describes the evolution of the Norwegian municipal chief executive officer (MCEO) from 1980 to 1992 and 2018, when the position was weakened politically but strengthened administratively. Today, the Norwegian MCEO corresponds to Mouritzen and Svara’s council–manager model, making it one of the most powerful in the Nordic region, second only to the Finish MCEO. Despite multiple significant contextual changes in the last two to three decades, the essential role of the Norwegian MCEO has remained remarkably stable, demonstrated in survey data from 1997 to 2017. Nevertheless, the data also show that the biographical characteristics of the MCEO have undergone considerable change during this period, most notably the increase in the proportion of female MCEOs from 7% to 30%. Further, today’s MCEOs are older and more highly educated, especially in disciplines such as economics, administration, and public management, reflecting the strong emphasis on economic management and performance in Norwegian municipalities. Finally, MCEOs are met with increasing expectations to combine innovation and management, cooperate internally and externally, and govern municipal affairs through complex, hybrid networks.

  In this study, we examine how organisational conditions impact boundary spanning in public health to handle complex tasks. This policy field is characterised by extensive pressure for collaborative governance and boundary-spanning activities. Data from a 2019 web-based survey of all Norwegian public health coordinators (n = 428; response rate 60%) demonstrated the importance of boundary spanning “by architecture” for completing complex transboundary tasks, such as local government health overviews. Combinations of organisational conditions; organisational size, position size, position in the organisational hierarchy and formalised network arrangements, affected degrees of boundary-spanning and the ability to complete health overviews. The most important indirect organisational condition seemed to be position size. Organisational size is an important organisational predictor for position size, position in the organisational hierarchy and collaborative partners’ contact pattern. Large municipalities had higher coordination capacity, higher degrees of boundary spanning and more formalised structures for intersectoral collaboration. Organisational size correlated significantly with contact frequency between boundary spanners and internal and external professional expertise. Overall, boundary spanning is not influential per se, but different degrees of boundary spanning affected the completion of complicated transboundary task in public health.

  Worksites are important settings for implementing health promotion programs. Evidence for sustainable upscaling of physical activity (PA) programs and critical evaluation of the implementation process are scarce. In this article, we address the following research questions: (i) To what extent is the implementation process of PA programs theoretically informed? (ii) What characterizes the implementation process of PA programs in theory driven studies? (iii) Which facilitators and barriers are identified in the implementation process and at what level? We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines. The databases Medline (Ovid) and Sportdiscuss (Ebsco) were searched for peer-reviewed original articles published in English (2000–2020), from a European, North American, New Zealand and Australian context. Reported implementation components and facilitators/barriers (F/Bs) were detected, interpreted and analyzed according to implementation theory. Appraisal of methodological quality on included studies was executed. Of 767 eligible studies, 17 studies were included, 11 of which conducted a theory-based process evaluation of the implementation. They implemented composite PA programs, at two or more levels with internal or mixed implementation teams. F/Bs were most frequently related to the implementation component ‘fidelity’, corresponding to organizational and implementer level, and the component ‘reach’ corresponding to program and participant level. Notably, only one study reported F/Bs on the socio-political level. Despite more frequent use in recent years of theory-based implementation, few studies reported implementation effectiveness. Major challenges regarding incoherent use of theoretical concepts and scarcity of empirically tested frameworks are discussed.

  : Building heavily on the Health in All Policies (HiAP) approach, Norway implemented the Public Health Act in 2012 to reduce social inequalities in health. Local public health coordinators (PHCs) at municipal levels were seen as tools to provide local intersectoral public health work. In this study, we examine factors related to intersectoral agency and if intersectoral work is understood as relevant to securing social justice in local policy outcomes. A national web-based survey in 2019 of all Norwegian PHCs (n = 428) was conducted with a response rate of 60%. Data were analysed through multiple linear regression, hierarchical regression modelling and structural equation modelling. Neither factors relating to community contexts nor individual characteristics were associated with intersectoral agency. Organisational factors, especially position size, being organised at the top level and having a job description, were significantly associated with perceptions of intersectoral agency. PHCs seeing themselves as intersectoral agents also found themselves able to affect annual budgets and policy outcomes. We conclude that municipal PHC positions can be important HiAP tools in local public health policies. However, organisational factors affect how PHCs perceive their influence and role in the municipal organisation and thereby their possibilities to influence local policymaking through intersectoral agency

  Inter-municipal cooperation seems to be gaining popularity in many Western countries, making it of pressing importance to better understand what factors that may contribute to the success of such arrangements. This article focuses on three inter-municipal cooperative arrangements in the field of child welfare in Norway deemed as successful, setting out to find common features across the three. Combining document-studies and qualitative interviews, the study manages to reveal three main factors explaining success: a sense of urgency, incremental processes, and political and administrative anchoring. The study also indicates that these factors interact, and thus cannot be viewed as isolated explanations. Furthermore, the potential negative effects on allocative efficiency are highlighted. As the success of inter-municipal cooperation at least partly seems to depend on the task in focus is “moved out” of the ordinary activity and anchored at a higher political and administrative level, it is argued that this may further fragment the municipalities’ ability to prioritize between sectors and services.

  Denne artikkelen tar sikte på å forklare policy-endring i en lokalpolitisk kontekst gjennom å kombinere tre teoretiske perspektiver: Regimeteori, en modifisert Garbage Can-modell og et lederskapsperspektiv. På 1990-tallet startet Kristiansand kommune en prosess for å selge sin aksjepost i det regionale energiverket. De frigjorte midlene ble brukt til å opprette to stiftelser. Stiftelsen Cultiva, opprettet av Kristiansand kommune, og Sørlandets kompetansefond, opprettet av kommunene i Vest-Agder, skulle drive regional utvikling gjennom investering i kompetansebygging, kunst og kultur. Denne typen satsing var ikke unik for disse kommunene, men utfallet i Kristiansand kan likevel klassifiseres som radikalt. For det første var det økonomiske omfanget unikt, for det andre representerte denne utviklingen et brudd med den borgerlige majoritetens mangeårige satsing på skole og helse. Denne artikkelen reiser spørsmålet om hvordan utfallet av disse beslutningsprosessene kan forklares. Artikkelen viser at koalisjonsbygging var viktig i prosessen, men at regimeteori ikke kan forklare prosessutfallet. I stedet blir en lederskapsbasert forklaring som vektlegger betydningen av kommunale topplederes rolle sentral. Den administrative og politiske toppledelsens suksess skyldes deres funksjon som både policy-entreprenører og aktører med tilgang til det formelle beslutningssystemet.   How did the Norwegian government succeed in the geographic relocation of a number of domestic agencies? This study suggests an organisational theory approach to explain the success. It argues that formal organisation of the decision-making process in government largely explains the success of the relocation reform. The government had to contend with: physical relocation of institutions tending to mobilise the attention and resistance of affected stakeholders, earlier attempts at physical relocation of agencies largely failing, and the absence of parliamentary majority. Studies demonstrate that large-scale reform processes tend to be characterised by medium degrees of hierarchical control. Instrumental leadership tends to be present in minor institutional reforms than in large-scale reforms. This study suggests that large-scale physical relocation of agencies was completed largely through the formal design of the reform process, i.e. by safeguarding hierarchical leadership from the government.

  Artikkelen presenterer et organisasjonsteoretisk perspektiv på fysisk relokalisering av sentraladministrative organer. Spørsmålet artikkelen reiser er hvordan regjeringen lyktes i geografiske flytting av statlige tilsyn. Argumentet som fremmes er at den formelle organiseringen av beslutningsprosesser i stor grad kan forklare utfall. På mange måter arbeidet regjeringen Bondevik mot mange odds: Fysisk relokalisering av institusjoner mobiliserer ofte mye oppmerksomhet og motstand fra berørte institusjoner, tidligere forsøk på utflytting av statlige tilsyn hadde i hovedsak strandet, og regjeringen Bondevik hadde ikke flertall bak seg i Stortinget. Tidligere studier viser at større reformprosesser kjennetegnes ved begrenset mulighet for styring og kontroll fra politisk-administrative ledelse. Innslaget av instrumentelt lederskap synes å øke i mer avgrensede omorganiseringer sammenlignet med store forvaltningsreformer. Denne studien viser derimot at utflytting av statlige tilsyn i stor skala ble sikret gjennom den formelle organiseringen av beslutningsprosessen.   To what extent does ICT create new challenges for the relationship between public service providers and users? With this paper we hope to stimulate a public discussion and increase the awareness of the main challenges for public services and their users in the information society. By pointing out some main characteristics of the development of the welfare state up to present day, we discuss how information and communication technology (ICT) has influenced both past challenges and will influence new challenges in the information society.By focusing on the relationship between ICT and social capital, the overall argument in this paper is that ICT may enhance social capital, both from horizontal and hierarchical points of view. This implies that the public sector is aware of and allows for the new challenges the sector and its users face in the information society. ICT may reduce transaction costs and thereby increase the possibility of establishing relationships at a lower cost and at a wider range. At micro level this may encourage face-to-face contact and thus increase the foundation and development of social capital. At macro level government and public services must meet demands such as developing and increasing transparency, access to information and resources to users through well-designed e-governance initiatives and programmes.

  Disappointingly few efficacious health interventions are successfully scaled up and implemented in real world settings. This represents an evidence-to-practice gap, with loss of opportunity to improve practice. Aiming to improve nutrition in the first 1000 days of life, we have combined four efficacious dietary interventions into a single adapted digital resource (Nutrition Now) for implementation in a Norwegian community setting. Nutrition Now targets pregnant women and parents of 0–2-year-olds with messages focusing on healthy dietary behaviours. Early childhood education and care (ECEC) staff are provided with pedagogical tools addressing healthy food exposure and child food acceptance. Objectives: a) evaluate the effectiveness of provision of the Nutrition Now resource on child diet and diet-related outcomes, with special attention to the influence of socio-economic position, b) gather information on the effectiveness of the implementation process to inform forthcoming scale-up and c) perform trial- and model-based economic evaluations. This is a hybrid type 1 implementation study, focusing on evaluation of effectiveness. A quasi-experimental design with pre- and post-tests, where one municipality gets access to the resource (n~800), while a matched non-equivalent control municipality (n~800) does not, will be used. Effectiveness will be assessed by examining e.g., diet outcomes, developmental outcomes, and feeding practices. The resource will be implemented in ECEC settings and made available to pregnant women and parents through the Norwegian system of maternal and child health (MCH) care. The implementation process includes iterative adjustments and implementation strategies from the implementation framework Expert Recommendations for Implementing Change (ERIC) informed by dialogues with stakeholders. Implementation outcomes (e.g., acceptability and adoption) will be assessed through questionnaires and interviews with parents, ECEC and MCH staff, with particular attention to ethnic diverse groups. Both within-trial and modelling-based economic evaluation will be performed. Nutrition Now will bridge the existing evidence-to-practice gap through rigorous scientific effectiveness evaluation of municipal scale up and inform subsequent county scale up. The study is the first to implement efficacious nutrition interventions in early life with potential for health improvement using technology to maximise the reach and impact of both parental and MCH dietary guidance and ECEC practice.

  Dette paperet tar sikte på å forklare institusjonelle nyvinninger i Kristiansand kommune. Tidlig på 1990-tallet, på samme tid som liberaliseringen av kraftmarkedet fant sted, begynte den lokale prosessen med å selge kommunenes aksjepost i det regionale energiverket. Utfallet av prosessen var radikalt. Kommunen etablerte en stiftelse med en kapital på 1,5 milliarder kroner med det som formål å satse på kunst, kultur og kompetanse for å bedre levekår og drive lokal og regional utvikling. Dette representerte et radikalt brudd med den tradisjonelle satsingen på skole og helse, som blant annet kom til uttrykk gjennom programfestingen til flere av partiene i flere valgperioder. Spørsmålet som reises i dette paperet er hvordan dette nye sporet i kommunepolitikken i Kristiansand lot seg gjennomføre eller mer konkret hva som kan forklare dette skiftet fra fordelingspolitikk til kulturorientert vekstpolitikk? Teoretisk kombinerer paperet et regimeteoretisk perspektiv, en modifisert form av garbage can og et lederskapsperspektiv. Paperet viser empirisk at pre-emptive makt var viktig i prosessen, men at regimeteori ikke fullt ut kan forklare prosessutfallet. I stedet blir en lederskapsbasert forklaring sentral, som vektlegger betydningen av kommunale toppledere som fungerer både som policy entreprenører og som aktører med tilgang til det formelle beslutningssystemet. Dermed ender vi opp med en forklaring som både vektlegger tradisjonell ?makt over? og mulighetsvinduer forårsaket av eksterne hendelser.

  Tilsynsordningen har vært lite deabttert i Norge, til tross for at den brer om seg som aldri før og stadig mer av tilsynenes virksomhet ser ut til å drives uavhengig av det overornedes departements instruksjonsmyndighet. Hva betyr dette for den politiske styringen og forvaltningens nøytralitet og lojalitet overfor den politiske ledelsen? Bakgrunnen for dagens utvikling går tilbake til 1980- og -90-tallet da vi fikk en mer prinsipiell rendyrkingsstrategi som del av statens økende markedsorientering. Det siste tiårets utvikling har imidlertid også vist at denne modellen har sine grenser. I tilfeller hvor uavhengigheten har blitt strukket for langt, har fullmakter blitt trukket tilbake. Det er fremdeles et betydelig mulighetsrom for å drive politikk og uavhengigheten har sine grenser.

  Dette paperet tar sikte på å forklare institusjonelle nyvinninger i Kristiansand kommune.
