  Healthcare organisations stand at a critical juncture, facing pressing challenges such as constrained budgets, growing demands, workforce shortages and heightened public expectations. Now, more than ever, there is a dire need for innovative solutions. Digital service innovation holds immense promise, offering the potential to improve health outcomes, reduce costs, enhance service quality and elevate patient experiences. However, the adoption of these innovations has been slow. The COVID-19 pandemic served as a catalyst, dramatically accelerating the implementation of digital innovations in healthcare organisations. This rapid transformation was a necessary response to unprecedented conditions. However, there are concerns that the momentum gained during the pandemic is waning, with innovation rates slipping back to pre-pandemic levels. This paper argues that we must harness the lessons learned from the pandemic to sustain and increase the pace of innovation, addressing healthcare organisations' urgent challenges. It aims to provide practical insights for healthcare managers at various organisational levels, drawing from a compelling case study of digital service innovation during the COVID-19 pandemic at a Norwegian hospital. Here, practitioners will find six actionable recommendations designed to inspire and empower them to drive innovation in a post-pandemic era. By embracing these insights, healthcare managers can lead their organisations toward a more resilient, efficient and patient-centric future. Now is the time to build on the strides made during the pandemic and transform the healthcare landscape for the better.

  While marginalized groups are increasingly involved in design processes, established methods and techniques require several cognitive and sensory abilities that may not fit when designing with people with intellectual disabilities. Earlier research on design of technology with people with intellectual disabilities has primarily focused on the technological outcomes and less on the techniques and adjustments made by facilitators. In this study, we explore facilitators’ experiences of supporting adults and younger adults with intellectual disabilities during participation in digital technology design. The facilitators and users in this study participated in the design of a digital self-reflective career tool and a digital transport support tool. We conducted individual interviews with facilitators who had participated in various design activities and thematically analyzed the data. The findings show that facilitators focus on adapting to individual needs, experience a process of personal development, and learned by doing throughout the design activities. We argue that while the role of facilitating was duty- and task-oriented and challenging, the facilitators also experienced several essential gains. Experienced gains include the development of design-related knowledge, professional development, and a change in attitudes toward people with intellectual disability. This paper provides recommendations for guiding facilitators, considering both structural and individual needs, and offers insight into lessons learned.

  Background: Research suggests that people with intellectual disabilities have a higher risk for cardiovascular disease than the general population. The aim of this study was to identify barriers for the prevention, diagnosis, and treatment of cardiovascular disease for people with intellectual disabilities. Method: We conducted individual interviews with relatives and general practitioners and focus group interviews with staff working at an assisted home facility, a cardiac ward, an obesity clinic and two rehabilitation centres (n = 33) in Norway. Inductive approach and thematic analysis were used to analyse the data. Results: We identified barriers on an individual and a structural level. The underlying reason for these barriers is that health problems, such as cardiovascular disease, are regularly overlooked as the condition of intellectual disability overshadows other possible diagnoses. Conclusion: This focus on intellectual disability rather than other explanations leads to shortcomings in the prevention, diagnoses, and treatment of cardiovascular disease in this group.

  Information systems (IS) research often seeks to deliver practical impact in addition to the traditional requirement for theoretical contribution. While an admirable goal, it is nevertheless a challenging prospect, as key questions remain around how best to facilitate a relationship between IS academic and practitioner communities. To explore this issue, we analyse multi-case study data from interviews with 24 IS practitioner doctorates, industry contact points, and senior IS academics who sought to create a joint field between academia and practice during their research. Our findings reveal several boundary spanning activities needed to traverse field boundaries and maintain the joint field's existence across the stages of proof-of-concept, proof-of-value, and proof-of-use. Building on insights from the work of Pierre Bourdieu, we further discuss how IS practitioner doctorates operationalised capital, doxa, and habitus to achieve varying degrees of practical impact in their work. Action-oriented recommendations are presented to support practical impact going forward including creolised messages and the mobilisation of capital to change inter-field relationships. By adapting Bourdieu's Theory of Practice to the engaged scholarship discourse in IS, we contribute new insights into how the academia-practice gap might be addressed.

  Background: Telemedicine in outpatient services for people living with human immunodeficiency virus (PLHIV) was scaled up during the COVID-19 pandemic as services transitioned to remote care. Many studies have reported on the challenges and advantages of telemedicine care during the pandemic. However, there is limited research on the provision of telemedicine human immunodeficiency virus (HIV) care beyond the COVID-19 pandemic, which entails different telemedicine components and focuses on ways to improve the telemedicine experience for patients. This study aimed to explore PLHIV’s perceptions of use, needs, and preferences related to a telemedicine solution for HIV care in an outpatient clinic in Norway. The telemedicine solution included a pre-consultation questionnaire, asynchronous digital messages, and video consultation. Methods: Qualitative interviews were conducted with 12 PLHIV. The interviews were analysed using thematic analysis. Results: We identified four main themes that covered the participants’ perceptions, needs, and preferences: (1) perceived usability, (2) maintaining confidentiality, (3) accommodating personal preferences, and (4) perceived usefulness. Some participants had difficulty logging into the telemedicine solution. Other participants suggested additional functionalities, such as picture sharing and access to test result. Telemedicine care enabled the avoidance of stigmatising clinic experiences, although a few participants reported concerns about confidentiality and data security. Accommodating personal preferences and needs in terms of the type of consultations (in-person or video) and frequency of visits was essential to the participants. With telemedicine care, participants felt more in control of their own lives, perceiving that it increased their perceived quality of life and saved them both time and money for travelling to the clinic. Conclusions: Our study identified several specific needs and preferences related to the assessed technical solution and the provision of current and future telemedicine care services. Nevertheless, the telemedicine solution was perceived as a usable, flexible, and person-centred approach to HIV care, contributing to accommodating the participants’ personal preferences. However, healthcare professionals need to ensure that individual requirements and preferences are consistent with evidence-based follow-up and supported by person-centred care. Thus, the practice of shared decision making is important in telemedicine care.

  Background Despite parental concern, few studies have investigated children’s experiences with school-based screening of growth deviations. This study aimed to explore perceptions of height and weight screening and associations with body size dissatisfaction (BSD) among third-grade children aged 8–9 years in central Norway. Methods In a cross-sectional study between November 2021 and April 2022, perceptions of height and weight screening and BSD were assessed individually among 209 children (49% girls) through researcher-assisted interviews. Results Most children indicated satisfaction with the screening by selecting a happy emoji, whereas only 1% indicated dissatisfaction, by selecting an unhappy emoji. However, 23%–30% selected a neutral emoji, indicating either neutrality or a response between satisfaction and dissatisfaction. Hvordan utnytter de mulighetene og hvordan håndterer de utfordringene? Dette er spørsmål som får økende oppmerksomhet i internasjonal forskning, men som er lite utforsket i norsk sammenheng. Denne artikkelen er basert på en studie der tre norske caser – en lege på YouTube og to psykologer på Instagram – undersøkes og sammenlignes gjennom tekstanalyse og intervjuer.   People are increasingly turning to social media platforms to acquire information and seek advice on health matters. Consequently, a growing number of qualified healthcare professionals are using social media as channels for public health communication. On platforms such as YouTube and Instagram, health workers can reach a wide and interested audience while applying powerful tools for presentation and interaction. However, such platforms also represent certain challenges and dilemmas when doctors and psychologists become health influencers. Who do they represent? What style of communication is expected? And what responsibilities do they have toward their followers? The present study contributes to the field of investigation by employing qualitative methods. It is based on three focus group interviews conducted with students enrolled in health-related study programmes at Norwegian universities. The paper asks: How do future healthcare workers perceive the social media practices of popular healthcare experts regarding the advantages and dilemmas associated with such practices?

  In this article, we explored the experiences of staff working in Norwegian community-based living facilities, caring for people with intellectual disabilities during the Covid-19 pandemic, and what self-regulatory strategies staff used to maintain health and care service delivery and contribute to organisational resilience. A qualitative design was utilized. Data were collected from September 2021 to April 2022 with individual and focus group interviews. In this study, 25 staff members participated. Thematic analysis was conducted. Five main themes were identified regarding the experiences of staff during the pandemic: Lockdown of facilities, social isolation of residents, lack of health follow-up, high sick leave rates and communication challenges. Only some of the strategies to mitigate pandemic-related challenges were based on current Norwegian infection control guidelines. Some strategies were successful while others were not and some were negligent and challenged the delivery of health and care services for the residents.

  Da covid-19-pandemien kom, innførte den norske regjeringen inngripende tiltak for å håndtere situasjonen. Kommunene fikk ansvar for å iverksette tiltak for å redusere smitterisikoen for personer som mottar kommunale helse- og omsorgstjenester. Personer med utviklingshemming som mottar kommunale helse- og omsorgstjenester, ble berørt av disse. I denne studien undersøker vi utfordringene som ledere og ansatte i tilrettelagte boliger for utviklingshemmede hadde i sin arbeidshverdag under covid-19-pandemien, og hvilke strategier de tok i bruk. Hovedutfordringer vi identifiserte, omhandlet nedstenging av boliger, sosial isolering av beboere, helseoppfølging av beboere, høyt sykefravær og kommunikasjonsutfordringer mellom ansatte. Resultatene viste at ansatte og ledere tok i bruk ulike strategier for å opprettholde tjenestetilbudet. Noen strategier lyktes og andre ikke, og noen kan forstås som uforsvarlige. Vi fant også at noen strategier baserte seg på gjeldende retningslinjer, mens andre tilpasset tjenestetilbudet på grunnlag av brukergruppens spesielle behov. Vi konkluderer med at tiltakene som regjeringen og helse- og omsorgstjenesten implementerte under covid-19-pandemien, ikke var tilstrekkelige for å sikre en resilient tjeneste for personer med utviklingshemming som bor i tilrettelagte boliger.

  The COVID-19 pandemic has created a worldwide state of emergency, triggering extensive digital innovation within healthcare institutions to recover from the disrupted services caused by the pandemic. The purpose of this research is to explore the phenomena of digital innovation during these extraordinary conditions and to understand the impacts during the pandemic and beyond. To do so, we conducted a systematic literature review by analyzing 130 research articles across research disciplines that were published during the pandemic. We found that the innovation processes were highly iterative and focused on rapid diffusion to address the urgent need for stabilizing and recovering disrupted services. This short-term perspective may result in adverse impacts beyond the pandemic, such as increased inequity. Moreover, we found that some environmental factors were highly adaptive to the pandemic, whereas others were less so. We suggest that organizations should focus on the latter when building resilience to future pandemics.

  We develop principles that facilitate socially inclusive design-oriented research with marginalized groups. Building on the recognition that the research process must be informed by theoretical perspectives about social inclusion, our effort begins with an empirical investigation of a multiyear research project that designed several IT-based solutions for people with intellectual and developmental disabilities. We treat the efforts to design each solution as a “case,” capture primary data from multiple sources, and analyze it in light of three facets of social inclusion drawn from prior work: self-determination, belongingness, and social capital. The findings are interpreted to derive five principles for a socially inclusive design-oriented research process: (1) respecting multi- perspective problem ownership and integrated solution design, (2) surfacing emic contributions to guide artifact design, (3) leveraging the support network to shape artifact design and refine research conduct, (4) customizing design-evaluate cycles with inclusive practices, and (5) pursuing authenticity in research collaborations. We elaborate each principle with connections to different facets of social inclusion, guidelines suggested by our empirical investigation, and a mapping against contemporary design-oriented research approaches. The five principles suggest key directions to facilitate a socially inclusive design-oriented research process when working with marginalized groups. The paper concludes with a discussion of implications for IS scholars, and pointers for using design-oriented approaches for greater social inclusion of marginalized populations.

  Information and communication technology has become an important aspect of everyday life, including community living and social participation. However, people with intellectual disabilities face several societal inequalities, including digital exclusion. Even if people with intellectual disabilities still do not have the same access as others, this group is increasingly involved in digital technology design activities. Although digital technology design activities can lead to several user gains, little is known about how such activities affect digital inclusion. Therefore, we explore whether and how participation in digital technology design activities can support the digital inclusion of young adults with intellectual disabilities. We interviewed seven young adults with intellectual disabilities about their participation in ten digital technology design sessions. We also collected reflective notes from eight support workers who participated in the same design activities. The interviews were analysed thematically. Thematic analysis generated four themes describing how participation in technology design activities can support the digital inclusion of people with intellectual disabilities: improving digital skills and knowledge, displaying skills and competence, increased interest in technology use, and influencing and adapting technology. Participation in digital technology design activities with support workers can provide new opportunities for young adults with intellectual disabilities and can help overcome several digital activity barriers. Designers and researchers should increasingly, and actively involve people with intellectual disability in digital technology design activities as it can support and promote digital inclusion.

  Background The coronavirus disease 2019 pandemic forced healthcare workers to use alternative consultation approaches. In general practice, the use of video consultations (VCs) increased manyfold as countries were locked down. This scoping review aimed to summarize scientifc knowledge concerning the use of VC in general practice and focused on (1) the utilization of VC in general practice, (2) the experiences of the users of VC in general practice, and (3) how VC afected the clinical decision-making of general practitioners (GPs). Methods A scoping review was conducted in accordance with the methodology of Joanna Briggs Institute. Review questions were formulated to match each focus area. A three-step search strategy was employed to search scientifc and gray literature sources. MEDLINE, Embase, Scopus, OpenGrey, Google Scholar, and ClinicalTrials.gov were searched from 2010 to March 11th, 2021, and the search was re-run on August 18th, 2021. The extracted data were deductively coded into pre-defned main themes, whereas subthemes were inductively synthesized. The data within each subtheme were analysed through descriptive content analysis and presented in a narrative synthesis. Results Overall, 13 studies were included after screening 3,624 studies. Most patients were satisfed with VCs. VCs were most suitable for simpler issues, often shorter than face-to-face consultations, and were more likely to be used by younger patients. GPs enjoyed the fexibility and shorter duration of VCs; however, they felt an unsatisfactory deterioration in the GP-patient relationship. Despite the loss of clinical examination, diagnostic assessment was mostly successful, with little fear of missing serious illness. Prior clinical experience and a preexisting relationship with the patient were important factors for successful assessment via VC. Conclusions Both GPs and patients can be satisfed with VC in general practice in specifc contexts, and adequate clinical decision-making is possible. However, disadvantages such as a diminishing GP-patient relationship have been highlighted, and the use of VC in non-pandemic settings is limited. The role of VC in the future of general practice remains unclear, and further research is needed on the long-term adoption of VC in general practice.

  Social media represent new arenas for health communication. Platforms such as YouTube, Instagram, Snapchat, and TikTok provide unique opportunities for health workers to build understanding, engagement, and trust among a broad and young audience. At the same time, such platforms demand of health workers a careful balance between social closeness and professional distance. The article is based on a case study in which the award-winning Norwegian psychologist Maria Abrahamsen’s practice of health communication on Instagram is studied through the lens of multimodal discourse analysis. Following a bottom-up method, the study starts with a close reading of a single Instagram post, where video and written verbal text interact closely. The entire account is then studied as a complex case of mediated health communication. Conclusively, after a discussion of genre implications, the article suggests possible solutions to key challenges concerning the followers’ engagement and trust.

  Transportation is an essential aspect of everyday life. For people with intellectual disabilities transportation is one the largest barriers to community participation and a cause of inequality. However, digital technologies can reduce barriers for transportation use for people with intellectual disabilities and increase community mobility. The aim of this scoping review was to identify and map existing research on digital technology support for independent transport for people with intellectual disabilities and to identify knowledge gaps relevant for further research. The authors conducted a scoping review of articles presenting digital technologies designed to assist in outdoor navigation for people with intellectual disabilities. The search yielded 3195 items, of which 45 were reviewed and 13 included in this study. The results show that while a variation of design elements was utilized, digital technologies can effectively support individuals with intellectual disability in transport. Further research should focus on multiple contexts and types of transportation, different support needs during independent travel, real-world settings, participatory approaches, and the role of user training to enhance the adoption of digital technologies.

  Rural residency is an independent risk factor for being overweight, but little is known about why this is so. The purpose of our study was to gain insight into what Norwegian rural children say about a rural diet in comparison to an urban one. Child-friendly methods were used. We found a discrepancy between what the children said they eat – traditional, ‘healthy’ foods, and what they ate – largely ultra-processed foods. We explored this by using the frameworks of imagined foodways and biopedagogies. Their imagined foodways were rooted in notions of ‘traditional food’, connected to surrounding nature and a history of farming and hunting. Urban people were perceived as eating an inferior diet of very unhealthy and ultra-processed foods of which they do not know the origin of. As a result, the children rejected specific health- and nutrition recommendations that were incompatible with their notions of traditional foods, leading to a more calorie dense diet than their urban counterparts. Our research adds to knowledge on what role a rural diet can play on the prevalence of overweight in rural areas, and how biopedagogies can be recontextualized within different cultural fields.

  This paper describes the initial stages of the process of implementing the data infrastructure required to develop analytics capabilities in a public sector organization. Helfo (the Norwegian Health Economics Administration) is responsible for making payments to healthcare actors who submit reimbursement claims. An important task for Helfo is also to prevent and detect errors, and the organization is currently strengthening this capacity though employing data analytics and ar-tificial intelligence. Implementing data analytics entails more than a “plug-and-play” process, and we analyze the initial stages of the implementation process as a sociotechnical change process. As a starting point we employ the CRISP-DM process model and enrich this with additional steps and tasks that was found to be central in the case. We describe in more detail the preparatory work relating to the technical setup and data infrastructures, and the implications for the information processing routines of the organization more broadly. The case study shows that also the early-phase improvements in data access and utilizing basic analytics capabilities yielded instant benefits to the organization, even before employing advanced analytics and artificial intelligence.   Implementing national public healthcare information infrastructures are often challenging, this is even more so in the global south. In this paper we offer an analysis of a successful development and introduction of a national healthcare information system. The MIPRES was developed by the Colombian Ministry of Health as a web application for the prescription of high-cost technologies (drugs and devices), but over time grew to become the backbone of a renewed digital healthcare information infrastructure. Through a qualitative empirical study, informed by information infrastructure theory, we document the emergence and evolution of MIPRES and analyse both the contextual conditions and strategic choices that led to its success.

  We investigate the organizational capacities required to leverage digital infrastructures both (1) in re-sponse to crisis and external threats, and (2) in realizing the transformative potential associated with the digital infrastructures. Thus, our research question is: What is required for organizations to be able to transform in the face of disruptions and breakdowns? We report from an empirical study of a digital infrastructure innovation process in the context of the COVID-19 pandemic, which involved extensions and novel development of both the technology and the former service model. While the literature on organizational resilience offers us a conceptual framework to identify organizational capabilities, we lean on literature that foregrounds transformability as a crucial aspect of resilience. We discuss organ-izational capacities which are considered vital in realizing the potential for transformative crisis learn-ing in sociotechnical systems that builds adaptive capacity and influences the enactment of future or-ganisational routines.

  Background Obesity is a major health concern in western countries. In Norway, patients with obesity can attend weight-loss programmes, which focus on changes in dietary and physical activity habits. Use of self-monitoring is advocated when changing dietary and physical activity habits for adults with obesity. This study aimed to explore the experiences of patients with obesity who used activity monitors while attending a weight-loss programme. Methods Patients with body mass index (BMI) > 35 kg/m2 with weight related comorbidities or a BMI > 40 kg/m2 referred to an intermittent weight-loss programme were recruited into this study. They were introduced to one of three different activity monitors, Fitbit Zip™, Mio Fuse™, or Mio Slice™. Semi-structured interviews were performed with patients six months into the weight-loss programme. Thematic analysis was applied when analysing the data. Results Of the 29 informants (aged 21 to 66 years) interviewed, 59% were female. Their experience with activity monitors was related to their adherence to the weight-loss programme. Two main themes emerged from the informants stories: 1. “Activity monitors visualize proof of effort or failure to change health habits”. 2. “Activity monitors act as a positive or negative enforcer when incorporating change”. Conclusions Using activity monitors either strengthens or undermines patients’ attempts to change health habits when attending a weight-loss program. Our findings suggest a need for more individualized weight-loss programmes for patients with obesity.

  Enterprise architecture (EA) is a systematic way of designing, planning, and implementing process and technology changes to address the complexity of information system (IS) landscapes. EA is operationalized when architecture visions move towards realization through concrete projects. We report a case study on the dynamics of operationalizing EA in the Norwegian hospital sector by exploring different EA project trajectories. Our empirical context is an institutionally pluralistic setting where multiple logics coexist. We show that the distinct logic of EA is added to the institutional context and we find that tensions among existing medical, technical, and managerial logics and EA principles and assumptions emerge. We contribute to the under‐researched topic of EA operationalization by suggesting a model that demonstrates how the meeting of multiple institutional logics can lead to varying degrees of differentiation or even disassociation from EA visions during decision‐taking in projects. Furthermore, we advance extant research on IS projects' implementation in institutionally pluralistic settings by providing an empirical account of actors' interactions and project leadership arrangements that contribute to the persistence of coexisting logics in a dynamic equilibrium.

  This paper describes the initial stages of the process of implementing the data infrastructure required to develop analytics capabilities in a public sector organization. Helfo (the Norwegian Health Economics Administration) is responsible for making payments to healthcare actors who submit reimbursement claims. An important task for Helfo is also to prevent and detect errors, and the organization is currently strengthening this capacity though employing data analytics and ar-tificial intelligence. Implementing data analytics entails more than a “plug-and-play” process, and we analyze the initial stages of the implementation process as a sociotechnical change process. As a starting point we employ the CRISP-DM process model and enrich this with additional steps and tasks that was found to be central in the case. We describe in more detail the preparatory work relating to the technical setup and data infrastructures, and the implications for the information processing routines of the organization more broadly. The case study shows that also the early-phase improvements in data access and utilizing basic analytics capabilities yielded instant benefits to the organization, even before employing advanced analytics and artificial intelligence.
